Özet

Özet: Parkinson hastalığı (PH), substantia nigra pars compacta'daki dopaminerjik nöronların ilerleyici kaybına ve bazal gangliyon işlevindeki bozulmaya bağlı olarak gelişen; bradikinezi, hipokinezi, postural instabilite ve yürüyüş bozuklukları ile seyreden ilerleyici bir nörodejeneratif hastalıktır. Bu motor belirtiler bireylerin yaşam kalitesini olumsuz etkilemektedir. Bu nedenle egzersiz temelli rehabilitasyon yaklaşımları, Parkinson yönetiminde giderek artan bir öneme sahiptir. LSVT BIG (Lee Silverman Voice Treatment – BIG), büyük amplitüdlü hareketlerin yüksek yoğunlukta ve çok sayıda tekrarla uygulandığı, kanıta dayalı bir egzersiz protokolüdür. Motor imgeleme (Mİ) ise bir hareketin açık kas aktivasyonu olmaksızın zihinde prova edilmesi olup, gerçek motor hareket sırasında etkinleşen ağları aktive eder. Bu yapısal derlemenin amacı, primer Parkinson hastalarında LSVT BIG ile motor imgeleme eğitiminin birlikte uygulanmasının nörofizyolojik temelini değerlendirmek ve bu alandaki literatür eksikliğini ortaya koymaktır. Her iki yöntem de hareketin zihinsel temsilinin yeniden yapılandırılması ve gerekli amplitüdün düzeltilmesi gibi birbirini tamamlayıcı nitelik taşımaktadır. Motor imgelemenin ev ortamında ve düşük maliyetle uygulanabilmesi, LSVT BIG'in yoğun protokolüne ek bir seçenek sunabileceğini düşündürmektedir. Sonuç olarak bu derleme, gelecekteki randomize kontrollü klinik çalışmalara yapısal bir nörofizyolojik temel sunmaktadır.

Abstract: Parkinson’s disease (PD) is a progressive neurodegenerative disorder caused by the loss of dopaminergic neurons in the substantia nigra pars compacta, characterized by bradykinesia, hypokinesia, postural instability, and gait disturbances. Therefore, exercise-based rehabilitation approaches are of increasing importance in PD management. LSVT BIG is an evidence-based exercise protocol in which large-amplitude movements are applied at high intensity with numerous repetitions. Motor imagery (MI), on the other hand, is the mental rehearsal of a movement without overt muscle activation, activating the neural circuits engaged during actual motor execution. The aim of this conceptual review is to evaluate the neurophysiological basis of the combined application of LSVT BIG and motor imagery training in patients with Parkinson’s disease. Both methods act through complementary goals such as reconstructing the mental representation of movement and correcting movement amplitude. In conclusion, this review provides a robust neurophysiological framework for future randomized controlled trials evaluating combined LSVT BIG and motor imagery interventions.
