Özet

A 56 year-old man was examined at our clinic with complaint of dyspnea on exertion. His physical examination was normal. His electrocardiogram showed sinus rhythm with pathologic Q on derivations D2, D3, aVF. His chest-X-Ray revealed no cardiomegaly. The findings of 2D echocardiogram revealed a large inferior aneurysm of the left ventricle (LV) and within dense, spherical structure echoes with a diameter of 3.0x1,8 cm, which had a higher density than the myocardium (Figure 1 and 2). His angiogram showed significant proximal left circumflex artery lesion with nonsignificant lesions on left anterior descending and right coronary artery. We started anticoagulant therapy for 3 months. The term LV aneurysm is generally reserved for a discrete, dyskinetic area of the left ventricular wall with a broad neck. They occur approximately more often at the apex and in the anterior wall than in the inferoposterior wall. Inferior wall aneurysm within mural thrombus is almost rare. The present case is peculiar with large inferior aneurysm of the LV and striking thrombus.
