The Relationship Between Pain Threshold, Functional Status, and Psychosocial Status in Individuals with Knee Osteoarthritis and Chronic Low Back Pain

Özet
Objective: The onset of chronic low back pain (LBP) in individuals with knee osteoarthritis (OA) leads to a complex clinical presentation characterized by the interaction of pain, physical function, and psychosocial status. The purpose of this study was to investigate the relationship between pain, functional status, and psychosocial factors in individuals diagnosed with knee OA and chronic LBP.
Methods: A total of 40 individuals (30F, 10M) diagnosed with knee OA were enrolled in the study, 70% of whom were homemakers. Functional status were evaluated using the Lequesne Index of Severity for Osteoarthritis of the Knee, WOMAC Osteoarthritis Index, Lower Extremity Functional Scale (LEFS), 10-Meter Walk Test (10MWT), Oswestry Disability Index (ODI), the Activities-specific Balance Confidence (ABC) Scale, and Five-Times Sit-to-Stand Test (FTSST). LBP severity at rest, at night, and during activity was recorded utilizing the Visual Analog Scale (VAS). Psychosocial status was assessed via the Fear-Avoidance Beliefs Questionnaire (FABQ).
Results: The mean age of the participants with knee OA was 67.75 ± 9.61 years, and their LBP level during activity (VAS-Activity) was determined to be 5.85 ± 0.242. Moderate, statistically significant correlations were identified between the WOMAC-total scores and ABC Scale (r = -0.496, p = 0.001), FTSST (r = 0.459, p = 0.003), 10MWT (r = 0.386, p = 0.014), and LEFS (r = -0.549, p < 0.001) scores. Additionally, a moderate-to-strong statistically significant relationship was observed between the individuals' knee OA symptoms and LBP severity (p < 0.05). However, the relationship between functional status and FABQ scores was not statistically significant (p > 0.05).
Conclusion: The findings of this study demonstrate that OA symptoms are significantly associated with both lumbar and lower extremity functionality in individuals suffering from knee OA and chronic LBP. Furthermore, this study highlights the importance of using integrated treatment approaches in patients with chronic LBP accompanied by knee OA.

Amaç: Diz osteoartriti (OA) olan bireylerde kronik bel ağrısı (LBP) başlangıcı; ağrı, fiziksel fonksiyon ve psikososyal durumun etkileşimi ile karakterize karmaşık bir klinik tabloya yol açar. Bu çalışmanın amacı, diz OA ve kronik bel ağrısı tanısı alan bireylerde ağrı, fonksiyonel durum ve psikososyal faktörler arasındaki ilişkiyi incelemektir.
Yöntem: Çalışmaya diz OA tanısı konmuş toplam 40 birey (30 K, 10 E) dahil edilmiştir (%70 ev hanımı). Fonksiyonel durum; Lequesne İndeksi, WOMAC İndeksi, Alt Ekstremite Fonksiyonel Ölçeği (LEFS), 10 Metre Yürüme Testi (10MWT), Oswestry Yetiyitimi İndeksi (ODI), Aktiviteye Özel Denge Güven Ölçeği (ABC) ve Beş Kez Otur-Kalk Testi (FTSST) ile değerlendirilmiştir. İstirahat, gece ve aktivite sırasındaki bel ağrısı Görsel Analog Skala (VAS) ile; psikososyal durum Korku-Kaçınma İnanışları Anketi (FABQ) ile kaydedilmiştir.
Sonuçlar: Katılımcıların yaş ortalaması 67.75 ± 9.61 yıl, aktivite sırasındaki bel ağrısı 5.85 ± 0.24 bulunmuştur. WOMAC toplam puanları ile ABC Ölçeği (r = -0.496, p = 0.001), FTSST (r = 0.459, p = 0.003), 10MWT (r = 0.386, p = 0.014) ve LEFS (r = -0.549, p < 0.001) arasında istatistiksel olarak anlamlı korelasyonlar saptanmıştır. Diz OA semptomları ile bel ağrısı şiddeti arasında anlamlı ilişki gözlenmiştir (p < 0.05).
Tartışma: Diz OA semptomları hem lomber hem de alt ekstremite işlevselliği ile yakından ilişkilidir ve bütüncül rehabilitasyon yaklaşımlarının önemini ortaya koymaktadır.
